HPESC fee schedule

 

Please note that the cost for euthanasia DOES NOT INCLUDE AFTERCARE. For ACO, you still have to add the ACO pick up fee. None of the prices include travel fees (when applicable), off hours/emergency/assistant fees or Paw Prints.

 

Euthanasia Only (NAC):

 

<100lbs: $530; 2 pets same visit: second pet $450

100-149lbs: $560

150-199lbs: $580

200+: $620

 

Communal Cremation:

 

<50lbs: $240 (total with euthanasia= $770)

50-99lbs: $275 (total with euthanasia= $805)

100-124lbs: $320 (total with euthanasia= $880)

125-149lbs: $400 (total with euthanasia= $960)

150-199lbs: $450 (total with euthanasia= $1030)

200+lbs: $600 (total with euthanasia= $1220)

 

Individual Cremation:

 

<50lbs: $370 (total with euthanasia= $900)

50-99lbs: $430 (total with euthanasia= $960)

100-124lbs: $480 (total with euthanasia= $1040)

125-149lbs: $560 (total with euthanasia= $1120)

150-199lbs: $650 (total with euthanasia= $1230)

200+lbs: $800 (total with euthanasia= $1420)

 

Private Cremation (Aquamation or Fire):

 

$2200 (does not include euthanasia, travel, after hours, etc…)

* See addendum on Private Cremation package

 

Private Cremation and viewing:

 

Fire cremation:. We refer them to West Coast for the family to make arrangements with the crematory directly for pick up, cremation and visitation. They pay West Coast directly and we perform Euthanasia NAC (No Aftercare) and we leave the remains on their premises.

Aquamation: Viewings are available. We take care of the pet in the normal manner. The arrangements for Viewing/Wake are made directly with Honor Pet.

 

Traditional Fire Cremation additional fees:

 

Please note that these fees are in addition to the regular corresponding fee of regular cremation.

 

Individual Fire Cremation:

 

0-50lbs: additional $110

50-100lbs: additional $165

101-150+: additional $275

 

 

 

Aftercare only (pick up fee):

 

$375 Plus cost of communal or Individual or Private as above.

 

Special pick up:

 

$250 This is if the owner does NOT want the pet to sit in our cold storage and wants Crematory to come pick up on SAME DAY basis. (or early the next day if euthanasia is done late in the day). Arrangements with crematory have to be made at the time the appointment is made.

 

Off-Site fee (for Beach, Park, etc… visits)

 

$150 in addition to the normal fees

 

Hospice/QoL:

 

Regular price Hospice or QoL: $570

If all forms are NOT filled out and received ahead of time: additional $165

Follow up visits within one month with updated Hospice care history form: $470

Follow up visits within one month with no updated forms: Additional $165

Follow up visits over a month with updated Hospice visit History form: $570

Follow up visits over a month with NO Hospice visit history form update: additional $165.

 

Extended visit fees:

 

Visits that last for over one hour due to clients taking longer than usual will be assessed a $150 fee per 15 minutes over 60 minutes.

 

Paw prints:

 

Basic Paw print: $75

Basic with engraving: $90

with finish: $160

Deluxe: $195

 

Assistant fee:

 

Regular hours: $100 per assistant

Off hours: Additional $200 per assistant. We may not able to offer assistance to lift for emergency off hours calls. We might be able to offer assistance to lift off hours if scheduled ahead of time, depending on availability of assistants.

 

Please follow our safety policy for lifting:

 

In order to safely plan for lifting your pet to the doctor’s vehicle, the number of able people required, in addition to the doctor are: Pets under 75lbs : 1 person. Pets 75-99lbs: 2 people. Pets 100-l39: 3 people. Pets 140-175lbs: 4 people. Pets 175lbs and over: 5 or more (rule of thumb: no more than about 35lbs per person to minimize risks of injury).

 

For Pot Bellied Pigs and for giant breeds, regardless of weight, a minimum of 5 people in addition to the doctor are required.

 

Off hours fee:

 

M-F 4PM: $150

M-F 5:30PM: $200

M-F 7PM: $275

M-F 8:30PM $375 (if doctor is available and willing)

M-F 10PM: $500 (if doctor is available and willing)

M-F 11PM or later: $600 (if doctor is available and willing)

 

Saturday 8AM-3:30PM: $125

Sunday 8AM-3:30PM: $175

 

Week-Ends evenings:

4PM: $175

5:30PM: $250

7PM: $300

8:30PM: $400

10PM: $500

11PM+: $600

 

Please note that our after hours fee starts at 4PM.  The reason being that traffic is VERY difficult to navigate at that time and if the logistics of the route go against their location, it makes it very time consuming to navigate appointments during this slot.

 

Travel Fees

 

Please note that our travel fees are established not only based on distance, but also on difficulty of access and high traffic areas.

 

Approximately 0-30 miles :

 

No additional charge (includes all of Orange County plus some additional locations)

 

Includes the cities of Aliso Viejo, Anaheim, Anaheim Hills, Bellflower, Brea, Buena Park, Cerritos, Corona del Mar, Costa Mesa, Coto de Caza, Cowan Heights, Cypress, Dana Point, Diamond Bar, Downey, El Toro, Foothill Ranch, Fountain Valley, Fullerton, Garden Grove, Huntington Beach, Irvine, Laguna Niguel, Laguna Hills, Laguna Woods, La Habra, La Habra Heights, Lake Forest, Lakewood, La Mirada, La Palma, Long Beach, Los Alamitos, Midway City, Mission Viejo, Modjeska, Newport Beach, Norwalk, Orange, Pico Rivera, Placentia, Portola Hills, Rancho Santa Margarita, Rowland Heights, San Juan Capistrano, Santa Ana, Sante Fe Springs, Seal Beach, Signal Hill, Silverado, Stanton, Trabuco Canyon, Tustin, Villa Park, Westminster, Yorba Linda, , Walnut, Whittier.

Approximately 31-40 miles: $225

 

Includes the cities of Alhambra, Azusa, Bell, Bell Gardens, Carson, City of Industry, Chino, Chino Hills, Claremont, Commerce, Compton, Corona, Covina, El Monte, Gardena, Glendora, Hacienda Heights, Huntington Park, Jurupa Valley, La Puente, La Verne, Lynwood, Montclair, Montebello, Monterey Park, Norco, Ontario, Paramount, Pomona, San Clemente, San Dimas, San Gabriel, San Pedro, South Gate, Upland, Vernon, West Covina.

 

Approximately 41-50 miles: $400 *Note: Do not to schedule these calls earlier than 10:00 am or later than 1:00pm without the permission of the doctor. An additional Travel fee of $125 to be assessed outside of those hours during the week.

Includes the cities of Altadena, Arcadia, Beverly Hills, Burbank, Culver City, Culver Junction, Duarte, Echo Park, El Segundo, Fontana, Glendale, Grand Terrace, Hawthorne, Hermosa Beach, Hollywood, Lake Elsinore, Loma Linda, Lomita, Los Angeles, Manhattan Beach, Monrovia, Moreno Valley, Palos Verdes Estates, Pasadena, Rancho Cucamonga, Rancho Palos Verdes, Redondo Beach, Rialto, Riverside, Rolling Hills, Rolling Hills Estates, Rosemead, San Bernardino, San Marino, Santa Monica, Studio City, Temple City, Torrance, West Hollywood.

 

Over 50 miles: To be arranged individually, check with your manager first.

 

Additional areas with a travel fee REGARDLESS OF DISTANCE/MILEAGE:

 

Areas difficult of access and traffic issues:

 

Balboa Island: Make sure to map out Newport Beach Addresses or ask the client if they are on Balboa Island. That area is difficult of access and we will now charge an additional fee: $150

 

Laguna Beach: Due to the terrible congestion going down Laguna Canyon Road, or the detour necessary in order to avoid that road, a travel fee is assessed: $150

 

Anything west of the 710 except for Long Beach: $225

Anything west of the 110: $400

 

 

Urn Delivery fees:

 

0-30 miles: $95

31-40 miles: $180

41-50 miles: $225

51+ miles: we do not hand deliver. Arrangements have to be made to pick up from our office or ship via USPS. UPS does not accept ashes.

 

METHODS OF PAYMENT

 

We prefer Cash or Checks but CC are welcome if necessary. Checks are made payable to "Home Pet Euthanasia".

We accept Master Card, Visa, Discover and American Express credit cards.

If someone wants to give their credit card info in advance, or another person besides the client is paying for the services, these are the items needed:

 

Name on the card (spelled correctly)

Card #

Expiration date

3 digit code on back of card

Zip Code of billing address

 

DO NOT EMAIL CLIENT CC INFORMATION. CALL THE MANAGER TO GIVE HER THE CC NUMBER VERBALLY.

 

OTHER FEES

 

Cancelled appointments are subject to a cancellation fee if reasonable notice is not given (see our specific policy regarding this).

 

We understand that sometimes, an appointment may extend beyond a normal visit time. We will never rush clients but please be advised that an extended consultation fee may be applicable for longer-than-usual visits.

 

The cancellation/minimum visit fee is $375

 

IMPORTANT POINTS TO REMEMBER

 

Sometimes prospective clients will request Dr Forslund during another doctor’s scheduled work hours. If this occurs, please follow the instructions below from Annie. Tell the prospect that all of our doctors have been chosen for their compassion and caring and they are all very special and awesome. They have been trained by Dr. Annie personally and they and their pet will be very well cared for regardless of which doctor helps them.
